A Case of Wegener¡¯s Granulomatosis in the Temporal Bone

Abstract
Friedrich Wegener first described Wegener¡¯s granulomatosis in 1936. Necrotizing granulomatosis and vasculitis of the respiratory tract and kidney characterize Wegener¡¯s granulomatosis. The mean age of onset is approximately 40 year-old. It was once uniformly fatal, but today cyclophosphamide and prednisolone can achieve a 70% to 85% remission rate. Because otological symptoms are frequent complaints in this uncommom disease, many of these patients are initially evaluated by an otolaryngologist. Otitis media with facial paralysis has already been reported in several cases of Wegener¡¯s granulomatosis in the literature. We present one case with a history of otitis media with effusion followed by facial paralysis. The patient was diagnosed as Wegener¡¯s granulomatosis based on clinical, radiological and pathologic findings in spite of the negative result on cANCA (antineutrophil cytoplasm autoantibody).
